Here are the steps to becoming a patient:

  1. Click the button below to start the process of becoming a new patient.

  2. Check your email or text for a link to the patient portal after submitting your information.

    Note: The clinic is open Tuesday, Wednesday, and Thursday. We will reply as soon as possible on the days we are open. On some occasions, we may be able to reply on a Monday or Friday.

  3. Use the portal link and code to create your patient portal within 7 days (code expires after 7 days).

  4. In your MYIO portal, request your appointment and fill out the initial paperwork.
