Steps to becoming a patient:

  1. Required - picture of insurance card, if you’re using insurance

  2. Click the “New Patient Form” button below to begin

  3. Fill out the form, upload both sides of you insurance card

  4. Expect an encrypted email with detailed scheduling instructions from info@oakwoodclinical.com within 2-5 business days

  5. Call us if you have questions
