Here are the steps to becoming a patient:

  1. Click the button below to provide us with the information that will help us match you to a counselor

  2. Check your email or text for a link to the patient portal after submitting your information.

    Note: The clinic is open Tuesday, Wednesday, and Thursday. We will reply as soon as possible on the days we are open. On some occasions, we may be able to reply on a Monday or Friday.

  3. Use the patient portal link and code to create a patient portal account within 7 days (code expires after 7 days)

  4. In your patient portal, request your appointment and fill out the initial paperwork
